and/or make decisions on your behalf in the event of a disability.
This will ensure that if you are ever incapacitated, your designee
will be able to act on your behalf.
Living Will/Medical Directive: A Living Will/Medical Directive is a legal
document in which you designate another person to make medical care and
life sustaining decisions in the event you are incapacitated.
